Registration Information: Please enter names of all team members so we can easily identify teams. Payment is made SEPARATELY using the PAYMENT form where the option is given to pay for 1, 2, or 3 registration fees. If you are paying for members other than yourself, please include a note in the message box to let us know! ALSO: if you are NOT including a registration fee because someone else is paying, make a note of that for us too. Thank You!
